Name change request must be made via the Gateway.

Pharmacists licensed in other states may apply for reciprocity to Mississippi through the National Association of Boards of Pharmacy (NABP). Pharmacists licensed in Mississippi may also be eligible for reciprocal licensure in other states and must apply through NABP.

Background checks usually take approximately ten (10) - fourteen (14) business days after the card is received by the processing agency.

Yes. The Board does not accept CICS/FBI reports issued for other purposes or from another entity.

Yes. Applications cannot remain pending indefinitely.  If deficiencies are not timely corrected or if your application is denied or withdrawn, you must reapply.  Please make sure that all required documentation is submitted in a timely manner to prevent expiration of your application.

After an application is submitted and paid for online via the Gateway, the Licensing Division will mail the applicant a fingerprint card along with an instruction sheet. Fingerprint cards should be submitted to the address listed on the instruction sheet (ONLY after you have completed and paid for the online application).

If your license or registration has been revoked by the MS Board of Pharmacy, you must abide by the stipulations outlined in your board order before petitioning the Board for reinstatement. The Board order will specify any required mandated waiting period prior to applying for reinstatement.

No. The fingerprint card, form No. FD-258, must be submitted to the address listed on the instruction sheet and the $40.00 background processing fee must be paid online along with your application fee. Applications are submitted without the background check but are considered incomplete. All application items must be received before an application can be considered complete.

You will need a control substance registration if you are employed by a MS permitted pharmacy that ships control substances into MS.

Yes, all applicants must submit to fingerprinting and undergo a criminal history record check by the CICS. If your license or registration has been expired/inactive for two (2) or more years, you will be required to undergo a criminal background check to reinstate/renewal your license/registration.

No. The name on your license or registration must be your legal name.

A change of ownership application will need to be submitted if changes to the ownership are occurring at the parent level only of the ownership. Changes made at any other level (grandparent or great-grandparent) requires only a notice to be sent to licensing@mbp.ms.gov

Change of ownership applications and notices must be submitted within 15 days of the change.
